什麼是板機指?

厲家珍醫師指出,造成板機指常見的原因是手指反覆彎曲過度使用,導致肌腱與腱鞘因過度磨擦而產生發炎腫脹,手指彎曲後肌腱就卡住而手指伸不直。從解剖學來看,掌骨遠端處有一個類似滑輪的環,稱為「滑車(pulley)」,手指屈指肌腱通過此環,收縮時手指便彎曲。當手指過度使用,造成滑車與屈指肌腱過度磨擦,滑車會產生發炎、腫脹、狹窄的現象,伴隨屈指肌腱局限性增厚,導致肌腱無法在滑車內自由滑動。(延伸閱讀:保健『食』品還保健『藥』品?有吃有保庇的迷思與風險)
讓手指有休息的空檔
厲家珍醫師建議,當症狀較輕時,建議手指適度休息,避免過度使用,物理治療如按摩深層肌腱、牽拉肌腱、關節活動、熱敷、超音波、電療、蠟療等治療,或是使用副木固定,幫助手指在彎曲之後容易伸展,都可以幫忙緩解改善症狀。配合消炎止痛藥的治療,如果症狀仍持續,可以在局部注射類固醇。若症狀仍然無法改善,最後一步為手術治療,這是在局部麻醉下的小手術,將沾黏的軟組織切開,讓肌腱可以順暢的滑動。(延伸閱讀:經痛女性必看!營養師教妳平日喝「這個」緩解不適)
